Nothing fancy, but how could I have a post apocalyptic style game without a mutant two headed troll? Reminds of that old 1e chestnut – giant-two-headed-troll. This guy interrupted Isumi’s vigil when she buried her kin.

Multiattack. The mutant troll makes seven attacks: two with its bite and five with its claws.
Bite. Melee Weapon Attack: +7 to hit, reach 5 ft., one target.
Hit: 7 (1d6 + 4) piercing damage.
Claw. Melee Weapon Attack: +7 to hit, reach 5 ft., one target.
Hit: 11 (2d6 + 4) slashing damage.
