In the process of converting Scales of War. I’ll be starting my campaign from Garaitha’s Anvil – the 12th adventure in the series – skipping most of the build-up adventures as I have characters who are coming of Tomb of Annihilation and are level 14.
Ω Edit: Decision by my group to do a gothic horror campaign so will not be using these creatures… yet (I’m sure I’ll find a use for these guys). Other conversions will be incoming pending the kickoff of my Bloodborne/Steam Punk inspired campaign later this year.

Studied Foe. The fomorian painbringer gains a +2 bonus to attack rolls against a creature affected by its curse of the evil eye.
ACTIONS
Multiattack. The fomorian painbringer attacks twice with its flail or makes one flail attack and uses Evil Eye once.
Flail. Melee Weapon Attack: +9 to hit, reach 15 ft., one target.
Hit: 19 (3d8 + 6) bludgeoning damage and if the target is Large or smaller it must make a DC 17 Strength saving throw or fall prone.
Evil Eye. The fomorian painbringer magically forces a creature it can see within 60 feet of it to make a DC 14 Charisma saving throw. The creature takes 27 (6d8) psychic damage on a failed save, or half as much damage on a successful one.
Curse of the Evil Eye (Recharges after a Short or Long Rest). With a stare, the fomorian painbringer uses Evil Eye, but on a failed save, the creature is also cursed with magical deformities. While deformed, the creature has its speed halved and has disadvantage on ability checks, saving throws, and attacks based on Strength or Dexterity. The transformed creature can repeat the saving throw whenever it finishes a long rest, ending the effect on a success.
REACTIONS
Funnel Pain. The fomorian painbringer may use its reaction to cause a creature that hit it with an attack to suffer 18 (4d8) psychic damage.
Alternate Reaction:
Painful Visions (1/day). As a reaction when the fomorian painbringer suffers damage all creatures within 20 ft. sphere of him suffer half-the damage of the triggering attack as psychic damage. A DC 14 Charisma saving throw halves the damage.
