planestalker

In the process of converting Scales of War.  I’ll be starting my campaign from Garaitha’s Anvil – the 12th adventure in the series – skipping most of the build-up adventures as I have characters who are coming of Tomb of Annihilation and are level 14. One thing I don’t like about some 4e monsters is there are no descriptions or pictures of creatures.  This was especially the case in Dungeon during 4e.  This is an example and I just made something up suitable.

Ω Edit: Decision by my group to do a gothic horror campaign so will not be using these creatures… yet (Im sure I’ll find a use for these guys).  Other conversions will be incoming pending the kickoff of my Bloodborne/Steam Punk inspired campaign later this year.

PLANESTALKER
Githyanki knights called to eternal service by the Lich-Queen. The transformation turns them into beasts – with clawed hands, eschewing the weapons they used in life for a more brutal form of combat. Used by githyanki’s are both guardians and trackers to hunt down the enemies of the Lich-Queen.

Teleport. As a move action, the planestalker can teleport 25 ft. to a location it can see.

ACTIONS
MultiattackThe planestalker uses planephase, then makes two dread claw attacks.
Dread Claws. Melee Weapon Attack: +8 to hit, reach 5 ft., one target.
Hit:13 (2d8 + 4) piercing damage plus 13 (3d8) necrotic damage and the target is grappled (escape DC 18). Until the grapple ends, the planestalker cannot use its dread claws on another target. It may immediately use body snatchas part of this attack on a successful grapple.
Dimensional Disjunction (1/Day).The planestalker howls creating waves of discombobulating force out to 15 feet. This disruption sphere is centred on the planestalker and other planestalkers are immune to this effect.  Creatures that fail a DC 18 Wisdom or Charisma saving throw (targets choice) become vulnerable to the damage dealt bydread claws.

Planephase. As an action, the planestalker can partially phase into another plane. While in this form, the planestalker appears as a hazy shadow and gains resistance to all damage except psychic. This ability requires the planestalker to concentrate as if concentrating on a spell, and the effect ends if the planestalker fails a concentration check. The ability also ends instantly if the planestalker takes psychic damage.
Body Snatch.
 While the planestalker is planephased, it can attempt to shift into a pocket dimension with a grappled target. The target must make a DC 15 Charisma saving throw. On a failed save, the planestalker and its target are transported to a pocket dimension (20 ft. in diameter and 10 ft. high). Within the pocket dimension, the planestalker and its target are in the same phase and so the planestalker loses its resistance to damage. If the planestalker loses concentration on its planephase, both it and its target return to the location from which they left.

Reactions
Planar Evasion.When damaged by an attack, as a reaction, the planestalker teleports up to 15 feet. If the attack broke its concentration on planephase it instead teleports 35 feet.
